If you did the Cantu deal at Target and you are a Brand Club member, a Cantu survey will pop up after you sync your Brand Club account. So, you sync your account and then click on surveys. For me, the survey didn't show up until I synced my account a second time about 4 hours after I'd synced my account the first time after my purchase of the Cantu.
